An index of the professionals I work with, and the folio held on each. One ledger per person. Every ledger keeps its own source tags, verification queue and revision line — this page only points to them.

The speciality line is a working descriptor, not a regulatory title. Where a person’s credentials define what they may and may not do, the ledger itself is the authority — this page is only a way in.
Ruled slots for people whose ledgers are not yet opened. Fill the name, then replace the slot with a roster row.
Written down so the repository stays consistent as it grows past the point where you remember how you set it up.

The “confidential” marking states an intention; it does not create a control. Keep identity documents, account numbers, client names and anything you would not hand to a stranger out of this repository, and treat the folios as records of publicly verifiable facts plus your own notes.
